Method: A phenomenological–hermeneutical design with qualitative semi-structured interviews was used. A strategic selection procedure was chosen to recruit participants from physiotherapy clinics and a regional hospital in Norway. Nine cases of space-occupying lesions of the sacral bone are presented. The problems of the clinical diagnosis, which in many cases comes too late, are discussed together with the indications for surgical treatment in this special group of tumours. The main clues are provided by the changes in the X-rays as well as the more modern imaging techniques (CT and MRI). The surgical technique aims at a most radical tumour removal with preservation of the sacral nerve roots, after which stabilisation of the sometimes weakened pelvic girdle may be necessary. In dieser Kasuistik wird der Fall einer tuberkulosen Destruktion des rechten Sakroliakalgelenks eines 56-jahrigen Patienten vorgestellt. Auffallig sind die verspatete Diagnose durch Verwechslung der Beschwerdesymptomatik mit einer Lumbalgie durch Bandscheibenprolaps und die Abszedierung entlang der Psoasmuskulatur und in die umgebende intra- und extrapelvine Muskulatur. Die Therapie ist eine chirurgische, die durch eine einjahrige medikamentose Kombinationstherapie erganzt wird.

Bias in skeletal sexing is well known and depends upon the completeness of the skeletal remains. The problem is very hard concerning prehistorical remains. We applied ‘Visual methods’ and ‘Probabilistic Sex Diagnosis’ (DSP) on the coxal bones on the Epigravettian skeleton (ST1) from San Teodoro (Messina, Sicily). On the basis of cranial characters and the overall robustness of the postcranial skeleton, it was suggested, since the time of the discovery, a male sex for ST1. This determination was later modified on the basis of the pelvic girdle morphology, and the skeleton was attributed the female sex for many years by large consensus. The July 2003 Case of the Month (COM). A 62-year-old female patient experienced progressive muscular weakness over the last ten years, involving shoulder and pelvic girdle muscles, paraspinal and facial muscles. A biopsy was taken from the left deltoid muscle where hepatitis vaccination had taken place 4 weeks previously. The specimen revealed macrophagic myofasciitis due to the injection of aluminium-bound vaccines. The finding can be reproduced experimentally by injecting vaccines in rats. The pathomechanism is supposed to involve immune stimulation due to long term persistence of the adjuvant.
